Medical history form required for athletes

All athletes planning to participate in two-a-day workouts that start Monday must complete a required medical history form that is available online.

Students who do not have a physical exam on file from last season or who answer “yes” to any of the first six questions on the medical record form must have a new physical exam completed by Monday in order to participate in workouts.

“Anyone answering ‘yes’ to one of the first six questions must have a new physical exam,” coach Denney Faith said. “That also applies to anyone who is new to our athletic programs who doesn’t have a physical on file with us from last year. The incoming seventh graders will also have to get a physical completed before they start on the first day of school.”

The medical history form can be obtained from members of the coaching staff or downloaded from the school’s website. It can also be found on the Facebook page for the Albany Athletic Booster Club.

“This really isn’t anything new, but we usually handle it when we offered free physicals in May at the end of the school year,” Faith said. “We didn’t get to do that this year because the school was closed due to the coronavirus pandemic.”

Since students were not routinely sent a new medical history/physical exam form with the packet of consent forms mailed earlier in July, they will have to obtain the medical record form on their own and turn it in on the first day of workouts for both high school volleyball and football on Monday, Aug. 3.

Any student needing to get a physical exam can sign up for an exam at Action Sports Medicine in Abilene on Saturday, Aug. 1 from 9:00 .am. until 11:00 a.m.

Contact athletic director Ryder Peacock or coach Faith to reserve a place for the free exams.

“We are needing to keep a list of who plans go over to Abilene for a physical,” Peacock said. “We need them to let us know rather than just showing up.”

Physical exams can also be obtained locally through either of the two medical clinics.

High school band students, including incoming eighth graders, will also be required again to have a physical completed if they do not have one on file from last year or turned in with the sports packet on Monday.

Incoming eighth graders participating in band will be required to obtain one unless they had one on file last year due to athletic participation.

Both the Shackelford County Health Clinic and ResourceCare charge $25 for a school physical, and appointments are needed at both clinics.
